It was discovered that the causes of the stoppages were in fact attributable to several operating procedures and changes in system operating conditions over time. We then identified the causes & effects, allowing improved software reporting of process conditions, improved PM and symptom feedback which could avoid the circumstances leading to the stoppages. Operating procedures, reporting parameters and process communication were all revised, leading to elimination of the stoppages and maximising equipment uptime.
